      What information must agents provide to be verified?

      Cover Whale requires the following documents and information to verify agents:

      1. Agency name and address
      2. Federal Employer Identification Number (FEIN)
      3. Billing contact information and email
      4. State and license numbers of predominantly used state(s)
      5. Errors and Omissions (E&O) certificate with carrier name and policy
      6. Completed W9 form 
      7. Answers to the following questions:
        1. How did you hear about us?
        2. Are you a retailer or wholesaler?
