Operations

Requirements

  • DOT Number: Required.
  • Business Duration: All (new ventures now accepted)
  • Vehicle Age: 26 years or newer (vehicles older than 1998 are ineligible).
  • Tarping: Dump loads must be fully tarped; otherwise, Auto Liability (AL) is not offered.
  • Freeboard: Drivers must maintain at least 6 inches of freeboard or be ineligible for AL.
  • Data Gathering: Trucks loading or unloading at roadside construction sites are part of data collection but are not excluded.

Underwriting Review

Please provide pricing targets in the notes or any available quotes to help compare and adjust based on market intelligence. Loss runs are mandatory for quote reviews.

  • Binding Requirements:
    • Loss Runs: 1 year of Loss Runs valued within 30 days, required at time of bind.
    • Completed Application: Signed by both producer and insured, including but not limited to:
      • Previous carrier’s premium and loss history (prior year plus current/expiring year, valued within 30 days).
      • Detailed vehicle fleet and driver list.
      • Current MVRs (Motor Vehicle Records) for all drivers, valued within 30 days.
      • The signed Uninsured Motorists election/rejection form is binding.
      • Acceptable Compliance/OOS Scores with FMCSA.
      • Driver experience and driving records must meet eligibility criteria.
      • A mechanical inspection report is required for operations outside the 80% Vehicle Maintenance Compliance Threshold.
      • Documented safety program (if applicable).
      • Producer’s narrative risk description.
      • A detailed operation summary is in the “Summary” tab.

DriveSmart Program

Participation in the DriveSmart Program requires cameras or ELDs (Electronic Logging Devices). Many DSG haulers may lack ELDs, and relying on them could cause post-binding operational issues. Consider the operational impacts when selecting ELD versus cameras.
